Abstract
Nearly all older patients transitioning from hospital to post-acute home health care (HHC) use 5 or more medications (polypharmacy) that include at least 1 potentially inappropriate medication (PIM).1 PIM use worsens geriatric syndromes (eg, falls) and represents a leading cause of hospitalizations.2, 3, 4 Deprescribing is the process of discontinuing and/or reducing the dosage of PIMs,5 yet no effective approach to deprescribing has been examined in HHC.
