The costs of health care continue to increase, and the number of Americans who suffer from chronic disease continues to climb.1 Every year, thousands of Americans are hospitalized for complications of chronic illnesses that could be prevented with proactive management in the outpatient setting. The patient-centered medical home (PCMH) model is a novel approach to outpatient care that includes a health care team led by a primary care physician, emphasizes improving quality of care, uses technology to capture quality indicators, and focuses on whole patient care.2
