Abstract
Advanced practice nurses are in an ideal position to help lead the transitional care model. Evidence based practice (EBP) is at the forefront of ensuring good outcomes and safe care. Clinical nurse specialists are educationally prepared to promote the use of EBP within any care setting. The focus of this column is EBP translation in the skilled nursing facilities where there are proportionally fewer registered nurses, and much of the care is provided by licensed practical nurses and certified nursing assistants.
